"It's clearly a concern" — investigation underway after second workplace death
An investigation is underway by WorkSafe WA following a fatal crush injuries incident at Hofmann Engineering in Bassendean on 8 September, which is the second death to occur at the same workplace.
“It’s clearly a concern to WorkSafe that a second death has occurred at this workplace,” Acting WorkSafe WA Commissioner Jennifer Low said.
The incident involved a mature age apprentice in his 40s. The man was working near a steel object when it fell onto him, resulting in fatal crush injuries.
WorkSafe WA said WorkSafe inspectors attended the site on the afternoon of 8 September, shortly after receiving notification, and will continue to examine the incident scene and the circumstances that led to the incident.
WorkSafe WA has also confirmed that — over a previous worker death at the same workplace in 2019 — Hofmann Engineering was prosecuted and fined $567,000 in November 2024.
“WorkSafe will continue to conduct a thorough investigation to identify what caused this incident and whether any potential offences have been committed,” Low said.
“As always, once the investigation has concluded we will share any learnings with the industry as part of our commitment to supporting safe and healthy work practices in all workplaces.”
