Investigation report on submerged dozer incident

An investigation report has been published for an incident that occurred on 15 May 2011 in NSW. The incident involved a dozer driver reversing into water and becoming submerged while working at Mount Thorley Warkworth mine.

The investigation report is an extract from the original report prepared by the NSW Mine Safety Investigation Unit. The report is available as an information and education resource for industry to assist in preventing similar incidents in future.
